What is an equipment handover form?

An equipment handover form is a document completed every time equipment changes hands on a construction site. It creates a formal record that the equipment was handed to a specific person, in a specific condition, on a specific date — and that the recipient acknowledges receipt.

The handover form serves two purposes: it creates accountability (the person who signed acknowledges they have the item), and it creates evidence (if the item goes missing or comes back damaged, you have proof of its condition and holder at a specific point in time).

How to use this handover form effectively

One form per item, per handover

Complete a new form every time a piece of equipment changes hands — even if it's going back to the same person for the same project. Each form is an independent record.

Always document condition at handover

The condition notes section is important. When equipment comes back damaged, the handover form is your evidence of what condition it was in when it left. Without this, you can't prove who damaged it or whether it was already damaged.

Both signatures are essential

A form with only the issuer's signature is a note, not a record. You need the recipient's signature to create accountability. If the recipient refuses to sign, do not hand over the equipment until they do.

Keep originals in a single file

File the original form at head office or the depot — not on site, where it can be lost. Give the site a copy. The head office copy is your master record.
